Prominent Ear

Some people's ears stick out more than normal. This condition is sometimes referred to as "bat ears". Ears that stick out do not cause any physical problems, but they can make people feel self-conscious and can lead to children being teased.

About prominent ears

About one to two percent of the people in the UK think that their ears stick out too much. Ears are one of the first parts of the body to reach full size, which is why protruding ears can be more noticeable in children.

Treating prominent ears

The main treatment for prominent ears is an operation called pinnaplasty or otoplasty, that reshapes the cartilage (gristly tissue) in the ear and uses plastic stitches to pin the ears back (or sometimes just by using the stitches alone). The procedure may also be known as "ear pinning".

There is no medical need to have a pinnaplasty, but people with protruding ears often feel unhappy with their appearance.

Children are particularly vulnerable to teasing and name-calling, which can cause great distress.

Neck Lift

There are a number of procedures available to enhance the appearance of the neck. They range from very minimal in-office treatments with no down time, to more invasive procedures requiring anesthesia and time off for recovery. Aging of the neck is usually a combination of loose skin and muscle and some excess fat. What You Should Do After Chin or Jaw Augmentation Surgery?

Recovery from Jaw Operations

It is normal to experience some pain and difficulty talking and smiling for several days after jaw implant surgery. You should also expect to experience some bruising and swelling, especially if your surgeon used screws to help keep your chin or jaw implants in place. If you notice any signs of infection or experience an increase in pain, contact your surgeon immediately. Most people take a week off work to recover from surgery. Bruising generally improves within one week, but it may take longer for facial swelling to subside. It is also normal to experience some numbness around the treated area. Indications for Orthognatic Surgery

Excess mandibular bone causes protrusion of the lower jaw beyond the normal alignment with the upper jaw. This can prevent effective biting and chewing of food and hastens periodontal disease. Temporomandibular joint function and speech may be impaired. Excessive chin hyperplasia or malposition may require a reduction or sliding genioplasty. Retrognathia or micrognathism is a deficiency of the mandibular bone. This does not allow the upper and lower teeth to come together when chewing food and may affect speech. Sleep apnea may also be produced by a retruded or deficient lower jaw. A deficiency of bone supporting the chin may require surgery (genioplasty) to build up the tissue and provide a normal framework for the chin. Procedur in Orthognatic Surgery

In orthognathic surgery, an osteotomy (bone cut) is made in the affected jaw, and the bones are repositioned in a more normal alignment. Generally, the bones are held in their new positions with plates, screws and wires. The patient may also need arch bars placed on both jaws to add stability (a procedure called intermaxillary fixation). For patients with deformities in both jaws, simultaneous osteotomies may be undertaken to achieve correction. Patients with deficient bone or soft tissues of the face may require distraction osteogenesis. A distraction device is applied. Bone osteotomies are performed. The distraction device is then used to slowly apply a distractive force at the osteotomy site until new bone is formed. Once the desired reconstruction is achieved, the device is left in place until the bone is healed and then the device is removed. This enables a surgeon to elongate a facial bone and the adjacent soft tissue envelope. Patients with deficient bone tissue may require grafts from their ribs, hips or skull. Alloplastic replacement of missing bone may also be required. What is Orthognatic Surgery

Orthognathic surgery or Jaw Surgery is the surgical correction of skeletal anomalies or malformations involving the mandible (lower jaw) or the maxilla (upper jaw).

These malformations may be present at birth, or they may become evident as the patient grows and develops. Jaw malformations can cause chewing and eating difficulties, abnormal speech patterns, early loss of teeth, and disfigurement and dysfunction of the temporomandibular joint. Malocclusion (abnormal jaw relation) may be caused by a deficiency or excess of bony tissue in one or both jaws, or by trauma to the facial bones.

Recovery Period After Lower Eyelid Surgery

At first the incisions will probably be red and somewhat bumpy. Eventually, the resulting scar should become flat and inconspicuous. Your sutures (stitches) will be removed sometime within the first week. The swelling and discoloration around your eyes will gradually subside, and you'll start to look and feel better each day. Swelling and bruising varies considerably from person to person. Bruising typically disappears within seven to ten days. Within the first week you will be permitted to use makeup, if desired, to conceal any discoloration.

Your vision may be somewhat blurry for a few days or longer. Your eyes may be temporarily sensitive to light, and you may experience excess tearing or dryness. You may receive eyedrops to help relieve any burning or itching. For the first week, you’ll need to avoid activities that dry the eyes, including reading, watching television, wearing contacts, and using a computer. Also avoid excessive blinking, which leads to increased swelling. You should also wear dark sunglasses for a couple of weeks to protect your eyes from wind and sun irritation. The eyes may tire easily for several weeks; frequent naps are helpful. For the first three or four weeks, you should avoid any activity that increases blood flow to the eyes, including bending, lifting, crying and sports. Your surgeon will let you know when you are ready for exercise. Also avoid drinking alcohol, which can lead to fluid retention and delay recovery.

Expectation after Lower Eyelid Surgery

After surgery, the physician will lubricate your eyes with ointment to reduce dryness in this area. Your vision may be blurred temporarily from the ointment. (Ointment is not used after transconjunctival blepharoplasty.) Correction, ointment would be probably be used in both methods.

The first evening after surgery, you should rest quietly with your head elevated. It will help to apply cold compresses to your eyelids. (Avoid any compress heavier than one ounce. A Ziploc bag with a few frozen peas works well.) Your blood pressure should be monitored to avoid bleeding complications that can affect vision. And, although you can be up almost immediately, you should limit your activities.

How to Perform Lower Eyelid Surgery ?

This is how to perform lower lid blepharoplasty: 1. An incision is made along the lash line, smile creases 2. Excess fat, muscle and skin are removed 3. Fine sutures are used to close the incision 4. Permanent stitches will be removed 3-5 days after the procedure

If you have a pocket of fat beneath your lower eyelids, but do not need to have any loose skin removed, your surgeon may recommend a transconjunctival blepharoplasty. In this procedure the incision is made inside your lower eyelid, leaving no visible scar. It is usually performed on younger patients with fatty lower eyelids. Transconjunctival blepharoplasty does not tighten the skin, reduces the puffiness in the lower eyelid region.

Consultation before Lower Eyelid Surgery

During the consultation, you and your surgeon will discuss the changes that you would like to make in your appearance. He/she will explain the different options available to you, the procedure itself, and its risks and limitations. Technigue Upper Eyelid Surgery

The amount of excess skin to be removed is marked. The initial incision is made along the normal eyelid crease across the entire lid and extending slight upward and outward into the lateral canthus. The second incision is made a variable distance above the first, depending on the amount of skin to be removed. If the orbicularis muscle is thick or in excess, a small strip may be excised.

The orbital septum is then opened along its entire length, thus exposing the fat pockets overlying the levator aponeurosis (tendon of the eyelid opening muscle). The first or "middle" pocket is removed using a process of dissection, clamping, excision, and cauterization. The second or "nasal" pocket requires deeper dissection. Removal is done is a similar fashion. The skin edges are closed using either dissolving or permanent sutures. The latter seem to give a more predicable and finer scar.

Rhinoplasty's Complications

There are a number of risks and complications specific to rhinoplasty. They are: Red spots on your skin's surface. These are caused by burst, small blood vessels. The red spots are usually temporary, but they are permanent in some cases. Scarring - When rhinoplasty is performed from inside the nose ("Closed Rhinoplasty") the scars will be on the inside of the nose. The scars won't be visible, but you may be able to feel them for a while after surgery. Some surgeons utilize an "open" technique, where a small incision is make across the base of the nose known as the "columella." The columella is the vertical strip of tissue that separates the nostrils. This "open" procedure may result in a small scar on the underside of the nose. Revision rhinoplasty - In around ten percent of cases, the patient needs to have additional minor surgery to correct a slight deformity. After surgery, small burst blood vessels may appear as tiny red spots on the skin's surface; these are usually minor but may be permanent. As for scarring, when rhinoplasty is performed from inside the nose, there is no visible scarring at all; when an "open" technique is used, or when the procedure calls for the narrowing of flared nostrils, the small scars on the base of the nose are usually not visible. In about one case out of ten, a second procedure may be required-for example, to correct a minor deformity. Such cases are unpredictable and happen even to patients of the most skilled surgeons. Surgical Technique for Ethnic Rhinoplasty

Like other types of rhinoplasty surgery, ethnic rhinoplasty seeks to reshape the nose by altering the structure of the bone and cartilage beneath without undue external scarring. Some techniques are more commonly used for ethnic patients, though.
The flatter noses possessed by many non-Caucasian individuals can be addressed by grafting in extra cartilage from elsewhere in the body and shaping the skin of the nose tip more finely. Up to three incisions (at the base of skin outside each nostril and in the columella between them) may be used to narrow the base of a wider nose. Artificial implants may be inserted into the bridge of the nose to give it better definition as well. Small amounts of bone can be removed to eliminate humps. Asian's Nose Cartilage

The more delicate cartilaginous tissues of the lower lateral cartilage generally require reinforcement with autogenous cartilage from the ear or septum to obtain a desired result. Affecting a result with pure cartilage reshaping techniques is difficult and usually inadequate. Septal cartilage frequently requires two-layered reinforcement because of its thinness.
Generally, the lower lateral cartilage is too soft and pliable to adequately support the tip. Such softness precludes the successful use of onlay grafts to the tip, except for the rare patient who has sufficiently strong cartilage (about 10% of patients). While septal cartilage appears adequate for tip projection in closed rhinoplasty, the compromise of circulation at the tip when using the open rhinoplasty approach may contribute to long-term weakness, absorption of the septal cartilage, or both. Conchal cartilage grafts placed as a strut appear to have a more durable outcome. When tip grafting, defatting should be limited in order to decrease the likelihood of graft visibility. This is particularly true in the open approach, as the 2-layered graft combined with a columella incision can lead to compromised circulation at the tip area and more significant scarring. Problem Asian Rhinoplasty

Address the problem as isolated to the dorsum, tip, alar base, vertical dimension, or all of the above. The Asian nose shares similar ideal dimensions with the Caucasian nose but with emphasis on subtleness: the dorsum requires less height, the tip less definition, the alar base less narrowness.
As in Caucasians, ideally the radix begins at or slightly below the lash line. The length, measured from the idealized radix to the base of the columella (subnasale), occupies the central third of the face. The tilt of the columella measures 90-115° from the vertical plane, with higher angulation preferred for smaller women. Southeast Asians (Malay, Filipino, southern Chinese) typically require the most dorsal augmentation (4 mm or more), while northeast Asians (Korean, Japanese, northern Chinese) require less (1.5-2.5 mm) or none. It may be necessary to better define the tip and increase its projection. A deficient premaxilla may need augmentation, as evidenced by a retracted columella with deficient columellar show from lateral view. The nose may require lengthening as measured from the radix to the tip or from the radix to the base of the columella. The need for alar reduction is frequent in southeast Asians but much less so in northeast Asians.
